There are three forms needed for enrollment.

2010 Enrollment form .pdf format OR

2010 Enrollment Form .doc format

Health Form To be completed by Physician - must include immunization records or exemption letter. Contact us with questions. A copy of child's school medical record can be substituted for this form.

Medical History & Emergency Contacts Form To be completed by parent, must include three emergency contacts.

The Department of Health requires that all emergency and medical forms are complete and on file before campers attend their first day. If your child attended camp previously we may still have valid health information on file. Contact us to find out if your physician's form or immunization records need to be updated.

We accept checks, cash or Paypal. Payment details are on the enrollment form. Upon request, special payment arrangements can be made.
